Completing the Water Hauler "Add Equipment" Application
If you operate a Water Hauler business and are looking to add a new truck, trailer, or container, you can now complete this process through MiEHDWIS.
Once you complete your application in MiEHDWIS, it will be reviewed, and inspection requests will be sent out. Please confirm that all equipment is ready for inspection when your application is submitted. Equipment licenses will be activated AFTER an approved inspection.
Create New Equipment Licenses Application.
This can be completed in one of two ways through two methods:
Click on Create Activity from the Activity Menu located on the dashboard.
You can select your entity on the Activity Menu; if so, jump to step 3.
b. Choose Create Activity from the left-hand menu.
Select Water Hauler as the Entity Type.
Select Add Equipment Application on the Select Activity page.
Confirm your entity of the Select Entity page.
Your entity name should be listed under the Selected Entity title
To locate your entity, you can select:

If you are a contributor to a Water Hauler business, it should be listed here.

You can search for the Water Hauler business by typing in the business name.
Please do not create a new water hauler.
If you are starting a new water hauler business, please use the New Water Hauler Business Application Process Activity.
When ready, select the + Create button to create a new activity.
This will take you the Activity Overview page. Each Activity is assigned a unique ID number (ACT-XXXXXX)
Review the Guidance.
Please note, before you complete your application, ensure all new equipment is ready for inspection.
When ready, select Start Form within the Tasks card.
The application form will open within the MiEHDWIS system.
If you need to step away from this application, simply click on Save draft at the bottom of the page. This will allow you to save your progress and return at a later time.
Review and confirm the Business Information.
If the information is correct, select Correct and Complete.
If the information is incorrect, has changed, or you are serving new counties, select Inaccurate or Incomplete.
A new box will appear where you can list any corrections or changes, and list new counties served.
Review your Current Equipment.
If no information requires updating, please proceed to step 12.
If information needs to be changed:
Select the pencil icon to open the edit field.
If the plate number has changed, please add new plate number.
If the equipment status has changed:
Leave a comment regarding the status change.
Select Save.
When ready, add the new equipment to the corresponding section.
Select either, + Add Truck, + Add Trailer, + Add Set of Containers.
For Trucks, provide the following requirement information:
VIN Number
Truck Year
License Plate Number
Truck Make
Material
Capacity in Gallons
Model is optional
Select Save when information is provided. - Please note, you will need to enter all required information to save.
For Trailer, provide the following requirement information:
VIN Number
Truck Year
License Plate Number
Truck Make
Material
Capacity in Gallons
Model is optional
Select Save when information is provided. - Please note, you will need to enter all required information to save.
For Containers, provide the following information:
Name of Set of Containers
Quantity of Containers in the set
Capacity in Gallons
Container Materials
Select Save when information is provided. - Please note, you will need to enter all required information to save.
Add any Additional Comments you would like to provide to EGLE.
When the application is done, select Complete.
If you need more time or would like to submit your form at another time, select the Save Draft button.
Once you select Complete, the form will be automatically submitted to EGLE.
Before submitting your application to EGLE, please confirm that all equipment is ready for inspection. Equipment Licenses will be activated AFTER an approved inspection.
