Submitting a Bacteriological Sampling Plan
The Bacteriological Sampling plan stores your previously saved sample sites, Source Sample Sites, and Other Non-Distribution Sites in MiEHDWIS.
You don't need to provide new sites each year but may need to modify existing ones.
Create a new activity from one of the following options:
Option 1: Select Create Activity on the left main navigation menu.
Create Activity is located on the left side of your screenOption 2: Select Create Activity on the Dashboard’s Activity Menu card.
If you are linked to an entity, choose the entity from the drop-down menu and select Create Activity. You will be taken directly to step 3.
Select your entity from the drop-down menu and select Create Activity.
Select Community Water Supply as the Entity Type.
To filter by activity type, use the Category or Filter drop-down bar to narrow your selection.
Select Bacteriological Sampling Plan.
Choose the Entity (Water Supply) by one of the following options:
Option One (not linked to any Entities): Under Full Set of Community Water Supply Entities tab, enter the Community Water Supply by searching its name or Public Water Supply ID (PWSID, formerly WSSN, e.g., 6280).
Search by Name or PWSIDOption Two (linked to Entities): Under Your Community Water Supply Entities, confirm the correct Entity is selected.
Click the +Create button.
When the Bacteriological Sampling Plan Submission Activity Overview page appears, review the provided Guidance.
Select the Start Form under the Tasks section.
When the form appears, review the auto-populated data for accuracy.
If incorrect, contact your District Analyst (listed in the form).
Scroll down to the Distribution Sample Sites table and click + Add Sample Site.
A new area of the form will open and allow you to enter routine and repeat sample site information. Click Save.
To add additional sample sites, click the + Add Sample Site button again and repeat until all distribution sites are entered.
To edit an existing site, select the Pencil icon, to the right of the sampling point.
This will expand a section below the table for edit to be made to the selected sampling point.
Select Save when done.
To delete a sample Site, select the Trash Can icon.
Confirm you want to delete your site by selecting Delete.
If you do not wish to delete that site, select the Cancel.
Groundwater systems need to list all the source sample sites in the Source Sample Sites and Other Non-Distribution Sites table by clicking the + Add Source button.
The form will expand and allow you to type out the source sample site information. Click Save.
To add additional source sample sites, click the + Add Source button again and repeat until all source sites are entered.
To edit an existing site, select the Pencil icon, to the right of the facility point.
This will expand a section below the table for edit to be made to the facility point.
Select Save when done.
To delete a site, select the Trash Can icon.
Confirm you want to delete your site by selecting Delete.
If you do not wish to delete that site, select the Cancel.
You can enter general comments in the line below the source table.
Scroll down more and enter Laboratory Contact information and Public Advisory, Education, and Notification information.
Enter Other Distribution Methods (optional).
Then, click Complete.
Validate the addresses entered for Distribution Sample Sites, Source Sample Sites and Laboratory Contact address. The Entered Address appears above the Recommended Address. If no Recommended Address exists, that section remains blank. Select the Check Box next to the correct address.
Please note, you may need to scroll down to confirm all entered addresses.
Then click Continue.
If you have any maps or any other associated documents, please use the Upload Attachment button to upload the documents.
Review your activity.
Mark all generated Tasks as complete.
Newest assigned tasks appear at the top.
The Proceed to Certification button activates only after all tasks are complete.
Ensure all attachments are accurate.
You can edit file details, delete incorrect uploads, and add missing files.
After you click Proceed to Certification, the certifier must click Submit to EGLE and certify. Once done, the activity is officially submitted to EGLE.
Until the certifier approves and submits the activity to EGLE, you can edit file details, delete incorrect files, upload missed files, or withdraw the activity.

When the activity status changes to Submitted, you will know your submittal made it to EGLE.
Please do NOT create a new activity to fix any mistakes contained within a submitted activity.
Reach out to the appropriate DWEHD Staff Member via the Messages card to have the activity transitioned back to you for corrections.
More Information Needed
More Information Need means EGLE requires more details about your submission. EGLE will communicate with you through the MiEHDWIS system regarding what is needed.
Go to the Activity.
The More Information Needed status appears at the top of your activity overview.
Review the provided Guidance.
Review the Messages card for any notes or comments from EGLE staff regarding your submission.
When ready, select the Review Form button within the Tasks card.
This will reopen your sampling plan. Please make any edits or corrections needed.
When you are done, select Complete.
Optional: You may message EGLE staff back via the Messages card, letting them know you have accomplished your changes.
Select the Resubmit button, when you are ready to resubmit to EGLE.
