Submitting an Onsite Wastewater Management (OWM) request (LHD Staff)

Create a New Request
There are now two ways to create an activity in MiEHDWIS.
Click Create Activity from the left navigation menu.
On the Activity Menu card of your Dashboard, click the Create Activity button.
You will be taken to the Activity Menu, select Onsite Wastewater Management. Click on the Select Onsite Wastewater Management hyperlink at the bottom of the card.
Onsite Wastewater Management has two Activity Types available - OWM Request and OWM Training.
OWM Request is for Site-specific projects, proposals, and inquiries.
OWM Training allows local health departments to request training.
Select the appropriate Activity Type.
Locate the name of the Entity by:
Selecting the Your Onsite Wastewater Management Entities Tab and select the Entity type
Selecting the Full Set of Onsite Wastewater Management Entities and typing out the entity name
Once you have selected an entity confirm that that name is listed under Selected Entity.
OWM Request
If you selected OWM Request in Step 3, a new set of fillable and selectable options is now available to fill out.
Complete the form, including the required sections of:
Project Name.
Program Category.
Program Type.
Request Category.
Request Type
Use the plus button to add additional Request Categories and Types, if necessary.
Priority.
Address Line 1, City, State, and Zip Code.
Completing all other sections of this part is not mandatory; however, it is highly recommended, particularly for vacant parcels that lack an official address number.
Click Create. Move onto the Activity Overview section of this guide.
OWM Training
If you selected OWM Training in Step 3, a new set of fillable and selectable options is now available to fill out.
Complete the form, including the required sections of
Event Name.
Program Category.
Program Type.
Request Type.
Completing all other portions of this section is not required but strongly recommended.
The Requested Time Frame field allows users to select specific dates when the Requested Date option is selected.
Users can use the plus button to add additional requested dates.
Click Create. Move onto the Activity Overview section of this guide.
Activity Overview
After Creating the activity, you will be taken to the Activity Overview page.
Each activity is assigned a unique number (ACT-XXXXXX).
On this page you may:
use Upload Attachment to upload files.
review the Timeline.
complete any Tasks, if applicable.
To upload a file, click Upload Attachment.
Select the files to upload.
Update the File Name.
Provide an optional File Description.
Select the correct File Type.
To add more documents in Upload Attachment, click to browse or drag files to the top left of the card.
When ready, click Save. The files will be added to the Attachments card.
When satisfied, click Submit Request. A pup-up will confirm your submission. If you wish to proceed, click Submit Request again.
Once the request is submitted, you can contact the EGLE staff via the Messages card on your activity overview to provide additional information.
Additional Information
Clicking Submit Request changes your request status to Request Submitted. The EGLE staff will receive a notification about your submission.
To check your request status, return to the Activity Overview page and find Status at the top. You can also view your activity on the Activity History page. If you need to provide more information, you will be prompted to upload the required details and select Submit More Information.
