Completing the License Application
After creating your Temporary Campground, complete the License Application.
To finish your application, there are 3 steps.
Step 1: Completing the Application
On the Activity Overview Page, review the provided Guidance.
When ready, select the Start Form button located under Tasks.
This will open the application form.
Review the Event Organizer/Billing Information.
This information was provided when the Temporary Campground was created.
Confirm Event Details:
Event Name can be edited.
Event Description is not required but can be beneficial for your inspectors regarding the type of even you are hosting.
Event Start Date and Event End Date.
The maximum operation time of a temporary is 14 days.
There must be at least 30 days between events at the same location.
This form must be submitted at least 14 days in advance.
Please Note: the license may be extended up to an additional 14 days pending Local Health Department approval.
Provide Campground Details.
Number of campsites is required.
Please note: the minimum number of campsites for a temporary campground is 5.
The state license fee depends on the number of proposed campsites. The Local Health Department may charge additional inspection fees. Contact them for details.
Confirm you have notified the local municipality and provide the name of the municipality.
Confirm that you have communicated with your Local Health Department regarding your inspection fees.
Review the Site Layout Requirements
You will be required to attach a site plan during Step 2: Uploading Additional Documentation.
Provide information about required Utilities.
Drinking Water Sources
Select all drinking water sources for the campground.
For Well and Municipal Water Supply, provide the number of Drinking Water Outlets.
Must provide 1 drinking water outlet per 50 sites.
For Water Hauler Service Contract, it is required to provide the business name.
Click the + Add button and type the Water Hauler business name to select your contractor.
Please note: Some Water Supply types may require additional documentation.
Wastewater Management
Select how wastewater will be handled.
For Septage Hauler Contract, it is required to provide the business name.
Click + Add button and type the Septage Hauler business name to select your contractor.
Please note: Septage Hauler Contracts require additional documentation.
Garbage Collection
Provide the number of garbage containers for campers.
Sanitary Facilities
Facility and fixture requirements are in the Campground Administrative Rules:
For Primitive Campgrounds (No service building with water flushing toilets & no sewer connections at the site), see The Campground Rules for fixtures requirements for primitive campground privies (R 325.1555a Temporary primitive campgrounds; privies).
For Modern Campgrounds (Service building with water flush toilets and water under pressure, or a water outlet and sewer connection at each site), see the Minimum Number of Fixtures table.
Provide the number of toilets, urinals, sinks, showers, and/or privies for the Temporary Campground.
Optional: Applicant Comments/Notes
Please provide any additional information the Local Health Department completing the inspection on your Temporary Campground may need.
Review the Document(s) Required. These documents will need to be provided after completing this form.
When ready, select Complete.
Step 2: Upload Additional Documentation
After selecting the Complete button, you will return to the Activity Overview page.
Review the Guidance.
When you are ready, review the list of tasks located in the Tasks card.
To upload the required documents, navigate to Customer Attachments.
Select Upload Attachment.
The File Upload window will open. Browse or drag files that need to be attached.
Once files are added, select the appropriate File Type for each document. Then select Save.
After attaching documents, return to the Tasks card and mark the tasks complete.
If you don't need to attach Water Sample Results or a Water Hauler service contract, mark those tasks complete.
When all required documents are attached and tasks are complete, select the Proceed to Payment button.
A pop-up window will appear. You will need to confirm that you understand that the Local Health Department may charge locals fees. All Local fees must be paid before an inspection can be scheduled.
Once you have confirmed, select Proceed to Payment.
Step 3: Pay the State License Fee
The final step of the license application is paying the State of Michigan on a secure site. After payment, you will return automatically to MiEHDWIS.
Contact your Local Health Department about additional fees, such as inspection fees.
The Activity Overview page will update.
Review the provided Guidance.
Optional: Download your Temporary Campground invoice, by selecting the Invoice button on the Fees Card.
This is not required but recommended for your record keeping.
Select the green Pay button to pay your invoice.
You will be taken to the MiEHDWIS Secure Payment Portal.
Do not close this window.
After making your payment, you will automatically return to the Activity Overview Page.
Select Next to begin your payment.
Provide the billing address information and your payment method. Select Next.
Review and confirm your payment information. Select Pay Now.
You will be directed back to your Activity Overview page in MiEHDWIS.
Select the Paid button to confirm your payment.
Congratulations! You have completed your license application.
The Local Health Department has been notified of your application. Once all local fees have been paid, they will review your application and supporting documentation. They will then complete an inspection before issuing any licenses.
You will be able to download and print your license directly from MiEHDWIS after your inspection.
