Dynamic Forms

Dynamic forms are used to collect data from external users. You can use them to both collect new information and have users confirm existing information. A form contains rows (“zones”), tables and configurable widgets. Questions in the form may be set to ‘required’ or not. Fields within the form may conditionally appear depending on the answer given to a question.

Using Form Designer

Form Designer is where MiEHDWIS dynamic forms are built, published and maintained.

You must provide form details, which include the name of the form, the program area where it will be used, the data tables that will be called for pulling and/or storing (binding) data, and indicate which Activity Types will use the form.
