  1. You will now fill in the information for your sample sites one at a time. Note that the default Facility ID (Site Code) will be DIST and should only be changed if your supply has multiple distribution systems.

    1. Each line with a red asterisk is a required line that must be filled out before you can add the site.

    2. Pay attention to the section labeled Other Location Identifier. This line is used for sites whose location cannot be accurately captured with just the street address. A common example would be adding an apartment number.

    3. Once you enter in the service line material, interior plumbing material, and structure type, the form will automatically determine the tier.

titleIs it accurate for the Sample Site Statis to be “New” when the site has been sampled previously?

Yes, ‘New' Sample Site status is only new in the context of creating a new form within MiEHDWIS. It doesn't mean that the sites haven’t been sampled before. We expect that every site first reported in the MiEHDWIS form will be submitted to EGLE as New, and that's ok. We can cross-reference the older paper form.

titleWhat Service Line Material do I choose when a site uses multiple materials at each portion of the customer's service?

If the service line contains lead and other materials, please just select lead as the Service Line Material.

titleIf pipe material is not known in a section of the service (primarily interior) - how is that to be entered?

Choose ‘Unknown’ for the Interior Plumbing Material field (see screenshot below).
(info) ‘Unknown’ is a permitted answer only for sites with a lead service line, and in the case of lead + other materials.

  1. After you have entered all the information for a site, click Save. If you still need more sites, then click + Add Sample Site again to enter another site. Repeat this process until you have the required number of sites.

  2. Once you have entered the required number of sites, click Submit on the bottom left.

  3. Once you click Submit, you are not done yet. The certifier for your supply must click Review Form and then certify. Once this is complete, you have officially submitted the sampling plan.

While filling out the form, you can choose to export the sample sites to Microsoft Excel by scrolling down to the Sample Sites section and clicking Export.

After submitting the form to EGLE, you can download a PDF version of the form by going to the Attachments section and clicking the download button.
